Series 4/2024

Thank you for your interest in registering for the Executive Certificate in Home Modification Program! This site allows you to register for any of the courses in Series #4/2024.

SERIES 4/2024 COURSE DATES: 

Course 1 – October 15 - November 1, 2024

Course 2 – November 5 - 22, 2024

Course 3 – December 3 - 20, 2024

Course 4 – January 7 - 24, 2024

Course 5 - February 4 - 11, 2024

Final Exam – February 12 - 19, 2024

REGISTRATION FEES:

You may register for one course at a time or register for all five Executive Certificate courses at once to obtain a Program Bundle discount. To qualify for the Program Bundle discount, payment for all five courses must be made with your initial registration.

Pricing: 

Individual Course Payments = $250 per course for Courses 1-5 ($1,250 if each course paid separately)

Program Bundle Discount = register for all five courses at once to receive a discount! 

• Courses 1-5 : $1,100 ($150 discount)
• Courses 1-5 AOTA Member: $950 ($300 discount)
• Courses 1-5 HMOTA Member:  $950 ($300 discount)
• Courses 1-5 Rebuilding Together Staff: $800 ($450 discount)

1. Complete and submit this registration page.

2. Use the payment portal link on the next page to finalize your registration.

 Your space will only be reserved once payment is made. Course capacity is limited and space is on a first-come, first serve basis. 

CANCELLATION POLICY: Once paid, refunds are only provided if the student notifies the Program Office at homemods@usc.edu 48-hours in advance of the course start date, or due to extenuating circumstances. In the unlikely event that the USC Leonard Davis School of Gerontology must cancel a course offering, students will be given the option to: a) enroll in the next course offering, or b) receive a refund for that course.
